The speed of sound

The prototype of the XO ODONTOSON 360 was developed by Lennart Goof. In 1962 he was still a concerto violinist, yet just a few months later he had quit his career and started developing dental instruments based on his knowledge of sonic waves and frequencies. In 1965 he developed his first dental instrument, ODONTOSON. His development was innovative as it used the magnorestrictive oscillation principle to make the instrument vibrate. The solution was simple, yet genius: a rod made of ferrite was placed into a coil and made vibrate using an electric current, which in turn brought vibration to the instrument.

A perfect core

One of the biggest challenges during development of the instrument was finding the right material for the instruments. The technology behind the scaler yielded a very high frequency of 42 kHZ, which most materials would not have been able to withstand. The only material that was suitable was titanium.

ODONTOSON 360, to this day, remains the only dental scaler on the market with handcrafted titanium instruments. The developed technology provides for a unique circular tip oscillation with a very small amplitude of 10 – 20 μm.

The gentlest scaling possible

“New patients coming to my dental office are surprised that the scaling procedure may not be painful”

I think that the most noticeable change after switching to XO ODONTOSON 360 was the reactions of the new patients coming to my clinic. When they show up for their first scaling procedure they are frequently stressed and expect the same pain they have experienced in different dental offices. They often request that I use an anesthetic. Using an anesthetic during the scaling procedure is tricky as scaling requires time and operation on all teeth. I purposely use less anesthetic to show my patients that not all scaling procedures have to be painful. Next time they come for a visit, they know that it isn’t that painful and they don’t request an anesthetic. It’s because they trust us. Furthermore, gentle scaling not only reduces pain during treatment, but also after-treatment tooth sensitivity.

I also use XO ODONTOSON 360 when dealing with periodontal disease. Periodical subgingival scaling and flushing with iodine solution allows me to significantly stabilize even advanced periodontal disease. After following this procedure for 10 years I think that iodine is the antimicrobial that gives the best results. It’s been a long time since I’ve had to do flap surgery.

“I don’t have to twist my wrist anymore.”

What I appreciate about XO ODONTOSON 360 is that it has very long, thin instruments. This allows me to have easy access to deep periodontal pockets. It works all around the tip and I don’t have to worry about keeping the scaler instrument perpendicular to the tooth surface. With piezo scalers, to avoid hammering the tip against the tooth surface and significant damage to enamel and dentin, I would have to either move my wrist into an unnatural position or frequently change scaler instruments. So XO’s scaler allows me to not only clean better and avoid damaging the enamel and dentin of my patients’ teeth, but also to protect my wrists and arms and work faster.

Another ergonomic advantage that I enjoy a lot is when I need to switch from lower to upper jaw. Instead of twisting the hand piece and working against the twist of the suspension I can twist the instrument in the hand piece. You can’t do this with scalers which have instruments that need to be screwed. My colleagues can never believe this can be so simple!

A special feature of working with XO ODONTOSON 360 is that the tip works fine even when more than half of it is being used! (This is due to the oscillatory movement and high frequency). As those instruments are slightly less effective, I use them supragingivally on patients who have very sensitive teeth.

Successful non-surgical periodontal treatments

XO ODONTOSON 360 has been indicated to be particularly effective for subgingival flushing with 0.1% PVP-iodine solution.

Read the guide created by Dr. B. Rosling for the treatment of advanced periodontitis using XO ODONTOSON 360:

1. The procedure can be started after examination of the dentition, gathering information from the patient and careful instruction in oral hygiene.

2. Treatment planning: Most levels of periodontal disease are possible to treat using the XO ODONTOSON 360. However, the outcome of the treatment will depend on the initial level of disease and the way XO ODONTOSON 360 is used. Treatment of a single quadrant at each appointment is the general procedure. However, in very severe cases and/or with patients who feel uncomfortable being in the dental chair for a long time, you may divide the dentition into sextants.

3. Always use anaesthesia to be able to perform a good treatment!

4. The time input for each tooth with pockets of 6 mm depth or more is between 6 to 10 minutes – the deeper the longer. 6 minutes for a 6 mm pocket, then add about 1 minute for each 2 mm. Start by adjusting the power and irrigation to maximum and work in a vertical direction with small horizontal movements of about 2-3 mm. When reaching the bottom of the soft tissue pocket be a little aggressive by pushing the tip towards the bottom. Use very light pressure against the root/tooth surface. It isn’t the pressure of the tip but the ultra-sonic energy that removes the bio film of microorganisms and calculus. Remember to apply light pressure and never work without moving the tip. Use a water solution of PVP-iodine 0.1%. Bleeding is a common response to mechanical debridement and is associated with the inflammation of the tissue rather than wounding because of the scaling procedure.

5. When you have finished the treatment, always give a prescription to your patient of 0.2% chlorhexidine for mouth rinsing twice a day for 2 minutes over 10-14 days. The patient will have difficulties performing adequate tooth brushing the first week. Reinstruct the patient in oral hygiene measures and make an appointment for the next treatment 4 days later.

6. If your patient experiences pain because of hypersensitive root surfaces give him/her fluoride dentifrice and fluoride solution for mouth rinsing.

7. After treatment of all teeth call the patient for re-evaluation after 3 months and repeat scaling in the same way for all 5 mm pockets with bleeding on probing. Recall the patient every three months in the first year and repeat check-ups. After this first year, recall the patient once every 3-6-12 months, depending on the healing and level of oral hygiene.
